These Terms & Conditions (“Terms”) apply to all services provided by Pureline (“we,” “us,” “our”), including DNA testing, mobile phlebotomy, specimen collection, lab submissions, and any related services. By scheduling or receiving services from Pureline, you agree to these Terms.

1. Nature of Services
Pureline provides the following services:
• Mobile phlebotomy and specimen collection
• DNA and paternity swab collection (legal and non-legal)
• Wellness, diagnostic, and specialty lab collections
• Specimen transport to partnering laboratories
• Provider-order testing processing
Pureline does not diagnose, treat, or give medical advice. All results must be interpreted by a licensed healthcare provider.

2. Service Eligibility
You must:
• Be 18 years or older, or have a legal guardian present
• Provide accurate identification when required
• Provide correct contact and health information
• Provide a valid provider order when needed
False information may result in cancellation of services.

3. Appointments & Access
You agree to:
• Provide safe access to the collection location
• Be on time for your appointment
• Notify Pureline of any changes at least 24 hours prior
Pureline may charge a travel fee or cancellation fee depending on location and service type.

4. Payment Terms
• Payment is due before or at the time of service unless otherwise arranged.
• Pureline does not store credit card numbers.
• All fees for collection, transport, and lab processing are final.
• If a lab rejects a specimen due to reasons outside Pureline’s control (e.g., medication interference, insufficient specimen, improper fasting), the client is responsible for additional collection fees.
Pureline is not responsible for reimbursement of laboratory testing fees.

5. Laboratory Processing
Once collected:
• Specimens are submitted to accredited laboratories.
• Pureline is not responsible for lab result accuracy, delays, processing errors, or interpretation.
• Turnaround times are estimates only and not guaranteed.
• All laboratory work is governed by the policies of the partner lab.
Pureline’s responsibility ends once the specimen is delivered to the lab under proper protocol.

6. DNA Testing Terms
For DNA, paternity, or legal testing:
• Proper identification is required for legal results.
• Chain-of-custody procedures must be followed for court-admissible tests.
• Non-legal DNA results are for informational purposes only and are not admissible in court.
• Results are released only to authorized individuals.
Any falsification of identity or documentation voids the test.

7. Client Responsibilities
Clients must:
• Provide accurate information
• Follow all instructions (fasting, hydration, medication guidelines)
• Disclose any medical conditions or risks
• Ensure minors have legal consent
• Inform Pureline of allergies (e.g., latex)
Pureline is not liable for complications resulting from withheld information.

8. Risks of Phlebotomy
Phlebotomy may involve normal, temporary side effects such as:
• Bruising
• Discomfort
• Lightheadedness
• Bleeding
You agree that these risks are understood and acceptable.
Severe complications are rare; however, Pureline is not responsible for conditions outside our control.

9. HIPAA & Privacy
Pureline complies with all HIPAA regulations.
Your personal and health information:
• Is kept confidential
• Is shared only with authorized parties
• Is used only for service-related purposes

10. Communication Consent
By using Pureline’s services, you authorize Pureline to contact you via:
• Phone
• Text
• Secure messaging platforms
This includes appointment updates, result notifications, and service-related communication.

11. Cancellation & Refund Policy
• Same-day cancellations may incur a fee.
• Travel fees are non-refundable once a technician is en route.
• No refunds for completed collections.
Laboratory fees are non-refundable unless the lab cancels the test by its own policy.

12. No Warranty
Pureline provides services “as is” without warranties, including:
• No guarantee of test outcomes
• No guarantee of lab processing times
• No guarantee of insurance coverage or reimbursement
All medical decisions must be made by your provider.

13. Limitation of Liability
To the fullest extent permitted by law:
• Pureline is not liable for lab errors, delays, or specimen handling once handed off to the lab.
• Pureline is not responsible for results, diagnoses, or medical outcomes.
• Any claim against Pureline is limited to the cost of the service rendered.
Pureline does not assume responsibility for acts beyond our control.

14. Refusal of Service
Pureline reserves the right to decline or cancel service due to:
• Safety concerns
• Incomplete documentation
• Aggressive or inappropriate behavior
• Suspicion of fraudulent activity

These Terms are governed by the laws of Massachusetts and applicable federal regulations.

By scheduling or receiving services from Pureline, you acknowledge that you:

• Agree to all policies listed
• Understand the limitations of Pureline’s services
• Accept responsibility for providing accurate information
